さいたま市桜区田島5-15-5 / Saitama city, Sakura ward, Tajima

The main statue is 大日如来 Dainichi Nyorai.

The temple was founded by 日宥 Priest Nichiyu (1669 - 1730) of the Nichiren Sect.

The temple also has 薬王院薬師堂 a Hall for Yakushi Nyorai.
